HIP PROSTHESIS

It was one of the most important medical advances in orthopedic surgery. And there have been innumerable materials used to make tests of this type.

The use of biological and inorganic materials became popular at the beginning of the 20th century. Using mainly interposition layers to improve the movement of muscle tissue, fat, fascia, etc. But they failed because of the unpredictability of the results and the residual pain.

The first major advance was that of cast arthroplasty introduced by Smith Petersen as an alternative to interposition arthroplasty. The procedure consisted of placing a glass dome on the head of the femoral, which although it gave quite good indications, would break after a few months, so more durable materials such as Bakelite, pyrex, were tried, even though they were also discarded due to their fragility. It was until 1938 when the implants were created sufficiently durable with a material called vitalium

The Judet brothers developed a new femoral head implant with a short stem. They arranged for this prosthesis to be made of acrylic, but later changed to materials such as nylon, vitalium or stainless steel.

The fragmentation of the acrylic material led to an intense tissue reaction that resulted in bone destruction. The great advance in arthroplasty was the design of metal stents with stems that were fixed inside the femur. The two most important ones were the Thompson and Moore models, developed by, as their name suggests, Fred Thompson and Austin Moore. These stems allowed the transmission of force to be made in the direction of the femur axis.

Even with these advances, there was the great problem of bone wear generated by friction of the stent. This is why we chose to make the other part of the joint, acetabular. The two made by metallic materials, together gave birth to the first total hip prosthesis.
